RECONNECT is an NHS-commissioned service that helps people leaving prison access the healthcare and support they need to improve their health and wellbeing.
Our aim is to improve access to healthcare and community support, reduce health inequalities and help individuals overcome barriers they may face when returning to the community.
We work with individuals before and after their release, providing personalised support to help them access the services they need. Referrals can be made up to 12 weeks before release and within 28 days following release, with support continuing for up to six months in the community, depending on individual needs.
Our service is voluntary, and we work closely with prisons, probation, healthcare providers, local authorities, charities and community organisations to ensure a coordinated approach to supporting people following release.
Our goal is to ensure that no one leaving prison faces barriers to accessing the healthcare and support they need to move forward with confidence, recognising that strong partnership working is essential to achieving this.
